Vision coverage is provided by Avesis. The Avesis plan covers your periodic examinations to determine any visual problems or other abnormalities. Vision care benefits include examinations and corrective lenses. The employee share of single vision coverage is $0.46 per month and the family vision coverage is $2.50 per month. Premiums are divided between the first two paychecks of each month.
If you use a participating in-network doctor, your vision services are covered at 100% with a $10 co-payment for the examination and a minimum $20 co-payment if materials are required. There is up to a $50 wholesale frame value ($100-$150 retail value).
If you choose to be treated out-of-network, you can go to a licensed optometrist, ophthalmologist or optician of your choice. Avesis will reimburse you up $35. Eye exams and lenses are covered once every 12 months, frames once every 24 months. Elective contact lenses are covered either in-network or out-of-network up to $130. Contact lenses are in lieu of lenses for your frames, you may not obtain both in the same 12-month period.
Lasik surgery is not covered by the plan, however in lieu of other services during the benefit year an employee may utilize an Avesis provider for discounts of 5% - 25% and obtain a one-time (lifetime) allowance of $150.
