Job Summary

Under general supervision, the Opioid Care Coordinator develops and implements programs to address needs of individuals with Opioid Use Disorder in the community by connecting the individuals to appropriate community services and treatment providers. Serves as the primary point of contact for individuals seeking assistance and works collaboratively with a network of service providers to ensure comprehensive care and support.

Job Responsibilities

Essential duties and responsibilities may include, but are not limited to, the following:

  • Develops and implements programs and initiatives to address the needs of individuals with Opioid Use Disorder (OUD) in the community.
  • Manages and coordinates incoming referrals from healthcare providers, community partners, social service agencies, local government agencies, and other organizations who have contact with individuals with OUD. Responds to calls outside of normal business hours as required.
  • Conducts initial assessments to understand the needs and circumstances of the referred individuals.
  • Facilitates comprehensive evaluations to accurately assess individual needs and ensures access to appropriate services. Provides continuous support by guiding individuals through the service system, coordinating multi-disciplinary efforts, and ensuring seamless transitions and integrated treatment and support services.
  • Enables access to additional support services such as housing, employment, education, childcare, and social services.
  • Provides referrals for individuals with a full comprehensive Substance Use Disorder/Mental Health evaluation so individuals can achieve a comprehensive plan for treatment and support services.
  • Maintains weekly or more frequent communication with individuals to assist in navigating services ensuring adherence to treatment plans. Provides ongoing support, including transportation to appointments and related service meetings, to facilitate engagement and continuity of care.
  • Collaborates with treatment providers to ensure care coordination with other needed services. May provide direct counseling services in limited circumstances.
  • In consultation with the Director, approves funding for treatment or other services as allowable by settlement funds.
  • Assists Director with management of settlement fund budgets ensuring accurate tracking, report, and analysis of program expenditures. Prepares detailed program reports and compiles data to support expenses.
  • Engages with and provides education and program information to referred individuals and their families, emergency departments, public safety agencies, community health partners, pharmacies, school districts, colleges/universities, or other related agencies.
  • Establishes and maintains strong relationships with service providers including but not limited to those in healthcare, social services, and community organizations.
  • Collects and analyzes data to monitor program outcomes and effectiveness.
  • Maintains accurate records of contacts, referrals, evaluations, services and outcomes, including documentation for individuals who refused services.
  • Completes comprehensive assessment and enters information into Community Services Network (CSN).
  • Participates in community meetings, coalitions, task forces, and internal committees as assigned by Director.
  • Demonstrates the ability to exemplify, by his/her actions, the County PRIDE philosophy.
  • Performs related duties as assigned.

Standards for Initial Consideration

Education, Training, and Experience Guidelines
Bachelor?s Degree in Social Work, Psychology, or closely related field AND five (5) years of progressively responsible experience in mental health services, substance use disorder treatment, case management, or social work; OR an equivalent combination of education, training, and experience. Masters Degree in Social Work preferred.

Knowledge of:

  • Applicable local, State, and Federal laws, codes, regulations, and ordinances.
  • Opioid Use Disorder and Substance Use Disorders.
  • Community resources and services.
  • Social service delivery systems.
  • Mental health and substance use evaluation.
  • Care coordination and case management.
  • Program funding opportunities and resources and various insurance coverages.

  • Working on multiple tasks simultaneously to meet deadlines.

Supplemental Information

LICENSE AND CERTIFICATION REQUIREMENTS
Must possess a valid Driver?s license. Requires use of personal vehicle.
Certified Alcohol and Drug Counselor designation required within one (1) year of employment.
Designation as a Licensed Social Worker is preferred.

PHYSICAL DEMANDS AND WORKING ENVIRONMENT
Work is performed in a standard office environment and in the field. May be exposed to irate persons and potentially contagious illnesses when interacting with clients. May require response outside of normal business hours.
