Job Summary

Under general supervision, the Family Health Coordinator works to ensure the health of the community by addressing public health needs of pregnant/postpartum people, children, and adolescents throughout the collaborative service area. Gathers and analyzes information related to access to healthcare in the community. Educates regarding the importance of health and dental care and the services offered through Title V and Medicaid. Convenes groups to identify needs of individuals and populations in the community. Connects clients to healthcare and oral health providers as well as other community resources. This is a grant-funded position. Grant guidance and annual action plans influence the activities of this community-facing position.

Essential duties and responsibilities may include, but are not limited to, the following:
Provides psychosocial services, including screenings for depression, substance abuse, and domestic violence, anticipatory guidance, follow-up, and referrals for pregnant/post-partum clients.

  • Maintains documents and files; updates database information to document program activities according to program timelines.
  • Supports family engagement and health equity by planning, organizing, implementing, and documenting meetings with program clients and families.
  • Develops, implements, and coordinates specific prevention, promotion, and education programs including Title V and Medicaid benefits; participates in community outreach activities.
  • Develops provider referral networks through outreach and relationship building; educates and encourages promotion of pregnant/post-partum care, lactation, developmental screening, oral health, and other relevant public health interventions.
  • Establishes and maintains linkages with local providers and community resources.
  • Coordinates care and facilitates access to health care, dental, and community resources for clients/families based upon needs identified by client or provider.
  • Educates the public on health care coverage for underserved and uninsured populations via outreach to the community, schools, faith-based organizations, priority populations, social media, and other community partnerships.
  • Schedules and completes Medicaid Presumptive Eligibility applications to determine eligibility for immediate, temporary Medicaid insurance coverage.
  • Conducts developmental and behavioral screening for children; follow up and monitors or provides service referrals as needed.
  • Participates in health planning and needs assessment activities to build program referral networks and determine project goals.
  • Provides care coordination services to clients; responds to inquiries regarding dental care services, information, and oral health requests; provides advice regarding oral health to organized groups and individuals throughout the community.
  • Tailors services to address client choice, preference, and special needs such as language barriers, low literacy levels, and hearing or sight impairment.
  • Engages qualified interpreters to communicate with clients in their primary language.
  • Orders and maintains an inventory of program supplies.
  • Maintains documents and files; updates database information to document program activities and direct services.
  • Attends and participates in a variety of internal and external meetings, committees, and organizations.
  • Participates in department efforts related to accreditation/reaccreditation, performance management, quality improvement, and workforce development.
  • Demonstrates the ability to exemplify, by his/her actions, the County PRIDE philosophy.
  • Performs related duties as assigned.

    Standards for Initial Consideration

    Education, Training, and Experience Guidelines Bachelor's degree in social work, counseling, sociology, psychology, family and community service, public health, health or human development, health education, or individual and family studies, or related field or associate's degree in nursing; AND two (2) years of related work experience required.

    • Pregnancy, child, and adolescent growth and development as well as postpartum health.
