2019 Community Health Improvement Plan

We are happy to announce the release of the 2019 Community Health Improvement Plan (CHIP) for the Scott County and Rock Island County Health Departments.

The CHIP is a product of efforts by the community to formulate goals, strategies, and an action plan to address priority issues identified through the 2018 Community Health Assessment.  The plan will guide health improvement activities for the next three years.  More information is available at:
