What is Hawki?

Hawki stands for Healthy and Well Kids in Iowa.  Hawki is free or low-cost health insurance for the children and teenagers of working Iowa families. Hawki offers two types of plans – a medical and dental plan, and a dental only plan.  The monthly cost is based on a family’s size and income - no family pays more than $40 a month and some families pay nothing at all.   • Doctor visits
  • Immunizations
  • Dental care
  • Hospital care
  • Surgery
  • Eye exams, eye glasses and contact lenses
  • Hearing services
  • Prescriptions
  • Mental health and substance abuse care
  • Specialist care
  • Chiropractic care

Does my child qualify for Hawki?

To qualify for Hawki, your child must:

  • Be an Iowa resident
  • Be under the age of 19 years
  • Have no other health insurance
  • Be a United States citizen or a qualified alien
  • Not be a dependent of a State of Iowa employee
  • Not be on Medicaid
  • Meet income guidelines (Check the Hawki income limits table to see if your children qualify).

How do I apply for Hawki?

  • Visit the DHS Services Portal to complete an on-line application
  • Print and fill out an application from the DHS Services Portal and mail it to: Imaging Center 4. PO Box 2027, Cedar Rapids, IA 52406
  • Call Hawki Customer Service at 1-800-257-8563 to have an application mailed to you
